What happened to Big Boss?
XOF Trojan Horse Incident - 1975
After Big Boss's chop took a blast and went down medic was in front of him took the shrapnel in the head, and both was in the coma. The medic was in the coma for 9 years, but Big Boss's coma is unknown.
Hospital Escape - 1984
Before medic gain his consciousness Big Boss had already fully healed, and keep an eye on the medic as Ishmael help him escape from the hospital. After the escape when the ambulance he drove away from the hospital with the medic crashed he gain he was help to disappear by the help of Ocelot. Now he has his new identity and his phantom, he just disappear and create the Outer Heaven.
*Compare to his phantom, Big Boss also don't have a scar on his face, and he still have both arms.

Operation Intrude N312/N313 - 1995
These incident in 1995 was planned and Venom Snake acknowledge about it, But no clue that he know that he will die in 1995.
The Cassette tape that was sent from Big Boss on the B side it's written Operation Intrude N313. Maybe Venom Snake know that he will be dead, or just acknowledge about Big Boss going to sent some agent to Outer Heaven.
